If submitting a copy of the GPA, it must be marked with the design application number associated with the original GPA

If claiming small entity status, submit this form in the prescribed format from Schedule-II of the Designs Rules. Indian entities must also provide evidence of MSME Act, 2006 registration, while foreign entities need an affidavit from the applicant or authorised signatory per Rule 42

Submit the original Priority document as required by Rule 15 of the Designs Rules.

If the original priority document is not in English, provide an authenticated English translation

If the applicant for the priority application in the convention country differs from the Indian applicant, submit the original Assignment.
